There's an underlayer in the head - the directions called for something
called stockingette. I've never heard of that, and I didn't want to search
all over JoAnn Fabric (taking Gabe to craft or fabric stores is awfully
unfun, and I try to keep trips to these very short). So I just used the foot
of a pair of sheer hose. You stuff that, tie string around it to create an
indentation for the eyeline, and stitch a bead on it to make a nose. Then
when you pull the knit fabric that's the skin over, you have a cute little
nose bump : )

The hair was a PITA - short boy hair is hard to do! I crocheted it, and I
suck at crocheting.

A Waldorf doll is a gender-neutral dolly, usually with an expressionless
face - just dots for the eyes and mouth. AT least that's how they always
look to me. They can be dressed any way you like, and of course I suppose
they could have long hair and be called 'feminine'. Anyway, my kids dress
all their dollies (male and female) and their stuffed animals in whatever
clothes they have that will fit, so I'm not sure I understand why it's
important to have a gender-neutral dolly.
