Default MOMS study

Well, at the beginning when they start asking your obstetric history,
it says "If you have been pregnant several times, please fill in the
subsequent questions according to your last pregnancy that resulted in
the birth of (a) live infant(s)." That is the "pregnancy in question."
I know it does get confusing at times, but as long as you keep that in
mind, the questionnaire does make sense. I just went through the
entire thing one more time to see for sure.

I can't log into it for some reason. Possibly you need cookies enabled?

I went to see about filling this out, but it said that
if you wanted to fill it out about a pg loss, you should
have a partner filling things out as well (can't see a
good reason for doing that). Presumably said partner
should not have had pg loss. Would anyone like to be
my partner?
