Since 2000, the agency has received more than 500 reports of psychosis in
children 10 and younger who took ADHD drugs, including Shire Plc's Adderall,
Eli Lilly and Co.'s Strattera and Novartis AG's Ritalin and Focalin, a staff
review found.

"Signs and symptoms of psychosis or mania, particularly hallucinations, can
occur in some patients with no identifiable risk factors," according to an
agency review dated March 3. The documents were posted on the FDA Web site
in advance of a panel meeting next week.

In one report, a 7-year-old boy on Adderall reported seeing people sitting
in a rocking chair and people coming into the house and stealing Christmas
presents. A 5-year-old boy reported seeing snakes and polka-dot alligators.

Allen, the Lilly psychiatrist, said assessment of psychotic episodes in
children requires a lot of training by clinicians. It is easy for adults to
misinterpret as a hallucination what may be a normal part of a child's
falling asleep.

Lilly sold $552.1 million worth of Strattera in 2005, a 17 percent decline
from 2004 sales of $666.7 million, according to Lilly's latest financial
statement. Since the drug's approval in the U.S. in November 2002, 3.7
million people worldwide have used the medication, Lilly said.

ADHD psychosis (or ADD psychosis) is a distinctive form of psychosis,
identified by Leopold Bellak and his colleagues, which co-occurs with
attention-deficit hyperactivity disorder (ADHD) and tends to be
treatable with typical ADHD medication such as stimulants (or some
antidepressants), but not with conventional or atypical antipsychotic
medication.

Although this condition does not appear in DSM-IV, and it is not widely
recognized, it has been detailed in several academic papers which give
a number of case examples1, 2, 3.
