"Results: Mortality was lower in the group vaccinated with any vaccine compared with those not vaccinated, the mortality ratio being 0.74 (95% confidence interval 0.53 to 1.03). After cluster, age, and other vaccines were adjusted for, BCG was associated with significantly lower mortality (0.55 (0.36 to 0.85)). However, recipients of one dose of diphtheria, tetanus, and pertussis or polio vaccines had higher mortality than children who had received none of these vaccines (1.84 (1.10 to 3.10) for diphtheria, tetanus, and pertussis). Recipients of measles vaccine had a mortality ratio of 0.48 (0.27 to 0.87). When deaths from measles were excluded from the analysis the mortality ratio was 0.51 (0.28 to 0.95). The landmark approach produced rate ratio estimates biased towards one. Biases increased with interval between data collection visits and incidence of vaccination, but were unaffected by the underlying mortality level. Survival bias increased with the proportion of dead children whose vaccination status could not be updated. The re-analysis of data from Guinea-Bissau changed the estimated impact of DTP from an 84% higher mortality using the landmark approach to a 37% lower mortality using the retrospective updating approach." I presume this refers to the Chingelput Trial? The conclusion was :- "It is likely therefore that BCG may offer protection against childhood forms of tuberculosis. It is prudent in view of the seriousness of tuberculosis in childhood that the practice of vaccination of infants may not be discontinued. " Current WHO advice is :- "Based on WHO recommendations, the Bacille Calmette-Guérin (BCG) vaccine is used once at birth in most developing countries to reduce the severe consequences of tuberculosis in infants and children." "In August 1995, the WHO issued a statement on BCG vaccination with the following recommendations 1 In countries where the prevalence and incidence of tuberculosis are high, BCG vaccination should be given to infants as soon after birth as possible, and in any case, within the first year of life....repeat vaccination is not recommended, as scientific evidence does not support this practice." Routine BCG vaccination in low risk countries is no longer necessary and the "Withdrawn" statement misattributed above actually refers to a German (not Indian, and not recommended in the report) decision some years later not to recommend routine infant BCG vaccination. The Germans have been one of the leaders in developing a more effective vaccine than BCG however and a genetically modified variant of BCG, VPM1002, started trials a month or so ago. Might some ant-vaccination campaigner have got their knickers in a twist and misread something perhaps? This appears to be part of Corties self selecting survey "results". The IAS is the "Immunization Awareness Society" of New Zealand, an anti-vaccination campaign group. April 2005 to be precise, reporting, on an anti-vaccination group website a meeting of anti-vaccination campaigners held in Stuttgart. |
