Hales Lookup Please
A friend of mine has Generalised Anxiety Disorder, but was not on (and did not
require) medication during pregnancy. Okey Dokey - here goes:
Venlafaxine is a new ssri antidepressant. It inhibits both serotonin reuptake & norepinephrine reuptake. Somewhat similar in mechanism to other antidepressants such as Prozac. Fewer anticholinergic side effects. In an excellent study of 3 mothers (mean age=34.5 yrs, 84.5kg) receiving Venlafaxine (225-300 mg/d) the mean milk/plasma ratios for Venlafaxine and O-desmethylvenlafaxine (ODV) were 2.5 & 2.7, respectively. The mean maximum concentrations of Venlafaxine and ODV in milk were 1.16 mg/L & 0.796 mg/L. The Cmax for milk was 2.25 hours. The mean infant exposure was 3.2% for Venlafaxine and 3.2% for ODV of the weight adjusted maternal does. Venlafaxine was detected in one of the seven infants, while ODV was detected in 4 of the 7 infants. The infants were healthy and showed no acute adverse effects. Pregnancy risk:C Lactation Risk:L3 Theoretic Infant Dose: 647.9ug/kg/day Adult Concerns: Nausea/Vomiting, somnolence, dry mouth, dizziness, headache, weakness Ped Concerns: None reported via milk, but no studies are available. Drug Interactions: Serious sometimes fatal reactions when used with MAO inhibitors, or if used within 7-14 days of their use. Alternatives: Sertraline, Paroxetine T 1/2 (HALF-LIFE) = 5 Hrs pk (Peak)= 2.25 Milk MW (Molecular Weight - less than 200 means easily passed to milk)=313 Vd (volume of distribution - how widely it's distributed in the body - lower than 1 is pref) =4-12 liters/kg PB (Protein Binding - the higher the better)=27% Oral (Oral Bioavailability)=92% pKa (the lower the better, pref 7.2 or less)=9.4 I don't really know anythng about any of this stuff but just posted verbatim what the book says in it's entirety.
