I read in the Blogsphere just the other day that the herb cohosh was
attributed to directly cause three deaths.

There is also dozens of reported deaths reported yearly due to the
use of suppliments containing herbs which contain ephedra, including
"ma huang" or Ma-Wong, and it is believed some deaths which are
reported a stroke or heart attack can be also attributed to the use
of herbs and suppliments.
